1. How The Program Works
The Proud Smile Dental Clinic Referral Program is designed to reward our valued patients for referring friends, family, and acquaintances. When you enrol in the program, you’ll receive a unique referral link or code. Each time someone you refer books and attends an appointment at our clinic, you will receive a reward of $50 AUD, issued via gift card.
2. A Referral Is
A referral is defined as a new patient who has not previously attended or booked an appointment at Proud Smile Dental Clinic. To qualify, the referred individual must:
- Book an appointment using your unique referral link or code.
- Attend the scheduled appointment at the clinic.
Referrals from existing patients or duplicate accounts will not qualify for rewards.
3. Rewards and Offers
Successful referrals will earn the referring patient a reward of $50 AUD. Rewards will be issued via gift card. Rewards are non-transferable, non-exchangeable, and cannot be redeemed for cash.
4. Issuing of Rewards and Offers
Rewards will be processed within 15 business days after the referred individual has completed their first appointment at Proud Smile Dental Clinic. To qualify, the referred appointment must not be cancelled or disputed. Participants must ensure their details are accurate and up-to-date.
5. Privacy
All personal information collected through the Referral Program will be handled in accordance with Proud Smile Dental Clinic’s Privacy Policy, available at https://proudsmile.com.au/privacy-policy/ Proud Smile Dental Clinic is responsible for the collection and processing of personal data under applicable privacy laws.
6. General
6.1. These terms govern the Proud Smile Dental Clinic Referral Program and form a binding agreement between the referring patients (“you”) and Proud Smile Dental Clinic (“we”, “us”, “our”).
6.2. The Referral Program is for inviting personal contacts only, including friends, family, and acquaintances.
6.3. Invitations sent to unknown individuals or used for commercial purposes are strictly prohibited.
7. Completed Referrals and Limits
7.1. A referral is considered complete once the referred individual attends their first appointment without cancellation or dispute.
7.2. Proud Smile Dental Clinic reserves the right to impose limits on the number of referrals per participant.
7.3. Referred individuals must be new patients; existing patients do not qualify.
7.4. Participants must ensure all personal information provided is accurate and up-to-date. Proud Smile Dental Clinic may request ID verification to confirm eligibility.
8. Restrictions
8.1. This Referral Program cannot be combined with other discounts, promotions, or offers.
8.2. Participants must not use unsolicited communication methods (e.g., spam) to invite others.
8.3. Referral invitations must not be shared on platforms associated with inappropriate or prohibited content.
8.4. Participants are prohibited from misrepresenting the Referral Program or creating misleading content.
9. Term and Termination
9.1. The Referral Program is valid until further notice. Proud Smile Dental Clinic reserves the right to modify or terminate the program at any time.
9.2. Referrals completed before termination will be honoured, subject to these terms.
10. Exclusive Relationship
10.1. Participation in the Referral Program does not imply any legal or business relationship beyond being a patient.
10.2. Participants are not authorised to act on behalf of Proud Smile Dental Clinic.
11. Privacy and Information
11.1. Participants must respect the privacy of individuals they refer and inform them of applicable terms and conditions.
11.2. Referrals must only be made with the referred individual’s consent.
12. Amendments
Proud Smile Dental Clinic reserves the right to amend these terms and introduce new conditions or incentives. Participants will be informed of changes via the clinic’s website or email communications.
13. Violations and Termination
13.1. Proud Smile Dental Clinic reserves the right to disqualify participants suspected of violating these terms or engaging in fraudulent activity.
13.2. Current or former employees, contractors, or affiliates are not eligible to participate in the program.
14. Disclaimer and Indemnification
14.1. Proud Smile Dental Clinic makes no guarantees about the availability or continuation of the Referral Program.
14.2. Participants agree to indemnify and hold Proud Smile Dental Clinic harmless from claims or disputes arising from participation in the program.
15. No Correspondence, Disputes, and Refunds
No correspondence will be entered into regarding rewards, and disputes will not be entertained. All benefits are issued on an “as-is” basis and are non-refundable.
16. Applicable Law
These terms are governed by the laws of Queensland, Australia. Any disputes will be resolved under the jurisdiction of Australian courts.
